How to Apply


Please note the hours of operation for our Customer Service Request Counter located at 40 College Street over the Holiday season:

December 18th to December 21st7 am to 4 pm
December 22nd7 am to 12 noon
December 25th and December 26thCLOSED
December 27th and December 28th7 am to 4 pm
December 29th7 am to 12 noon
January 1st, 2018CLOSED

The counter will re-open for our regular hours of operation of 7 am to 5 pm on Tuesday, January 2nd, 2018.

 

Requests for Collision Reports, Occurrence summary/confirmation letters, and lost passport reports can be made either in person at Police Headquarters or by mail.

In Person

Please attend our Customer Service Centre, located at Toronto Police Headquarters – 40 College Street, Monday through Friday between 0700-1700 Hours.  Please note, requests are not accepted at Police Divisions.

  • Request forms are available at the Customer Service Centre.
  • Please ensure that you bring the following with you
    1. Two pieces of government identification (at least one with a photograph)
    2. Applicable fee (refer to Fee Schedule or below)
    3. All fees are payable with Visa, Mastercard, Amex or Interac. Please note, we do not accept cash at our Customer Service Centre. You may also pay with a certified cheque or money order made payable to the Toronto Police Service.
    4. Please provide as much information as possible pertaining to the report.
      • Officer’s name and badge number
      • Report number
      • Date of occurrence/collision
      • Location – road, street name, or intersecting streets
      • Names of involved parties
      • Plate numbers of vehicles involved
      • Type of occurrence

 

By Mail

Requests can be submitted by mail.

  • Either send in a completed request form or write a letter indicating the nature of your request.
  • Please ensure that you include the following with your letter or request form:
    1. A photocopy of two pieces of government Identification (at least one with a photograph)
    2. Applicable fee (refer to Fee Schedule or below)
    3. All fees are payable with a certified cheque or money order made payable to the Toronto Police Service.
    4. Please provide as much information as possible pertaining to the report.
      • Officer’s name and badge number
      • Report number
      • Date of occurrence/collision
      • Location – road, street number or intersecting street
      • Names of involved parties
      • Plate numbers of vehicles involved
      • Type of occurrence
    5. Mail your request to the following address:

* The request processing fee is waived for out-of-town visitors (Not residing in the Greater GTA Area), Students (actively enrolled in a High School or Post-Secondary Institution), and Seniors.
